INFORMATION COLLECTION AND ITS USAGE
For Non Registered User Services, ezHealthTrack doesn't ask for any personal information such as email, name, age, gender, address, identification number, unless user chooses to provide the same. When Users (Registered and Unregistered ) use our Services, you can (depending on the Service you use) submit information by: Writing data with Mobile application or type-in data (such as when you register for our Services, or type a message to send to us or another user, or type a search request, or complete an optional form); Uploading a document, image or other data file; or ezHealthTrack is authorized on user behalf to retrieve and import information from another user or other third party on your behalf. General Information: You do not have to give us any personal information to visit the product information portion of our web site. No personally identifying information (such as your name, e-mail address or Social Security number) will be collected unless you choose to provide them to us. ezHealthTrack collects personal information when you sign up for a trial or paid subscription with the ezHealthTrack service. When you sign up, we ask for your name, email address, phone number and your company's demographic and financial information such as credit card information. When you sign up for a paid subscription, we will ask you to enter your credit card or ACH information. This information is encrypted on computer systems that are secured in a locked cage at a data center co-location facility rented by ezHealthTrack. ezHealthTrack automatically receives and records information on our server logs from your browser, including your IP address and the page you request. ezHealthTrack uses information for the following general purposes: products and services provision, billing, identification and authentication, services improvement, contact, research, and anonymous reporting. ezHealthTrack will send you strictly service-related announcements on rare occasions when it is necessary to do so. For instance, if our service is temporarily suspended for maintenance, we might send you an email. Generally, you may not opt-out of these communications, which are not promotional in nature. If you do not wish to receive them, you have the option to deactivate your account. If you wish to subscribe to our newsletter, we will use your name and email address to send the newsletter to you. Out of respect for your privacy, we provide you a way to unsubscribe by clicking on the link within the email. If you sign up for ezHealthTrack Patient Portal and elect to use the online patient payment service, your transaction will be processed by Stripe, Inc., and the information you provide to Stripe is subject to its privacy policy. Your credit card number and billing information will not be accessible by ezHealthTrack. IP Addresses IP Addresses are automatically reported by your browser each time you view a web page or use our service. Your IP address is not permanently stored in a way that is identified with your personal information. IP addresses may be used for various purposes, including: To diagnose or service technology problems reported by our users or engineers that are associated with the IP addresses controlled by a specific web company or ISP. To estimate the total number of users visiting ezHealthTrack from specific geographical regions. Cookies and Other Tracking Technologies A cookie is a small amount of data, which often includes an anonymous unique identifier that is sent to your browser from a web site's computers and stored on your computer's hard drive. We do link the information we store in cookies to personal information you submit while on our web site. ezHealthTrack uses both "session" cookies and persistent cookies. Web Beacons / Gifs ezHealthTrack uses software technology called clear gifs (a.k.a. Web Beacons) to help us better manage content on our web site by informing us what content is effective. These technologies are tiny graphics with a unique identifier, similar in function to cookies, and are used to track the online movements of Web users. In contrast to cookies, which are stored on a user's computer hard drive, clear gifs are embedded invisibly on Web pages and are about the size of the period at the end of this sentence. In some cases we tie information gathered by clear gifs to our customers personal information; an example would be tracking emails that have been opened by recipients which allows us to measure the effectiveness of our communications and marketing campaigns. 3rd Party Tracking The use of cookies by any tracking utility company is not covered by our privacy policy. We do not have access or control over these cookies. Tracking utility company may use session ID cookies and/or persistent cookies. Behavioral Targeting / Re-Targeting We partner with a third party ad network to either display advertising on our web site or to manage our advertising on other sites. Our ad network partner uses cookies and Web beacons to collect non-personal information about your activities on this and other Web sites to provide you targeted advertising based upon your interests. Protected Health Information When using the web site, you will never provide ezHealthTrack with Protected Health Information (PHI), nor will ezHealthTrack show any PHI.
INFORMATION SHARING AND DISCLOSURE
ezHealthTrack may be required to disclose personally identifiable information under special circumstances, such as to comply with subpoenas or when your actions violate the ezHealthTrack Terms of Service. ezHealthTrack does not share demographic information with any business partners. No personally identifiable information will ever be used without your permission (i.e., we will ask you before using a quote or testimonial). Third party vendors, including Google, show our ads on sites on the Internet. Third party vendors, including Google, use cookies to serve ads based on a user's prior visits to our website. Users may opt out of Google's use of cookies by visiting the Google advertising opt-out page. ezHealthTrack does not rent, sell, or share personal information about you with other people or nonaffiliated companies for promotional purposes except to provide products or services you've requested or when we have your permission. With your consent, we do share your name and email with carefully selected partners. If you would not like your information shared with these partners, uncheck the box when asked this option or notify us viaprivacy@ezHealthTrack.com. It may be necessary to share information in order to investigate, prevent, or take action regarding illegal activities, suspected fraud, situations involving potential threats to the physical safety of any person, violations of ezHealthTrack's terms of use, or as otherwise required by law. We will transfer information about you if ezHealthTrack is acquired by or merged with another company. In this event, ezHealthTrack will notify you by email or by putting a prominent notice on the ezHealthTrack web site before information about you is transferred and becomes subject to a different privacy policy. We use other third parties such as a chat service provider to provide customer service to you, and a credit card processing company to bill you for goods and services, an email service provider to send out emails on our behalf. When you sign up for these services, we will specify what personally identifiable information is being shared as necessary for the third party to provide that service.
Accessing, updating, or correcting your personal information
